EISEP Caseworker (Full Time)

EISEP (Expanded In-home Services for the Elderly Program)




  1. Acquires knowledge of EISEP regulations and standards
  2. Keeps updated on new regulations and standards set by State Office for the Aging
  3. Provides case management services to functionally impaired elderly.
  4. Screens individuals for program eligibility
  5. Conducts in-home psychosocial assessments to determine client need and financial status with client and caregiver
  6. Completes full assessments on computer
  7. Develops and implements comprehensive care plans
  8. Authorizes and arranges for home care through contracted Aide Agencies/Day Programs
  9. Authorizes for payment of bills by tracking services
  10. Monitors clients and services in accordance with state regulations
  11. Keeps up-to-date records and documents according to policy
  12. Acquires up-to-date knowledge of aging services and professional practices through reading and attending seminars, workshops and conferences
  13. Participates in staff meetings and case reviews in accordance to state regulations
  14. Completes all statistics and written reports in a thorough and timely fashion.
  15. Conducts outreach and public relation activities as requested
  16. Is aware of and actively supports the Agency's cultural competency goals, efforts, and activities
  17. Other duties as requested by supervisor



  • Bachelor's Degree in Human Services or related field OR Registered Nurse with one-year experience in the field of Aging.  (BSW preferred)
  • Must be able to tolerate and handle stressful situations in a calm and professional manner
  • Valid NYS driver license, clean driver’s abstract
  • Car necessary
